New Patient Registration

we cover the Towns of Uddingston and Bothwell. The general postcodes covered are G71 5, G71 6, G71 7 and G71 8. The map below will give a good idea of the area covered however please do call and ask as, due to the vagaries of area boundaries, there are some areas of G71 7 that we are unable to cover as they are within Greater Glasgow Health Board.



If you would like to join the practice, one of the patient service advisers will give you a registration form. Persons 16 and over shall also receive a general medical history form to complete. You will then be registered with the practice and may choose to consult with any of the GP partners.

All new patients who register with the practice must have an initial health check before registration is confirmed. This is a free health check for you and an opportunity for us to obtain important medical history prior to your medical notes arriving. This information will be gathered by using our Patient Services POD

You may print off a registration form and a new patient questionnaire , fill it out and bring it in with you on your first visit to the practice.  We also require two recent proof of residence documents (e.g. utility bill, bank statement, community tax).
